As a vital member of the team, your primary focus will be delivering effective and high-quality nursing care in the home environment. You'll play a crucial role in facilitating early supported transfers from hospitals and preventing unnecessary admissions, contributing to positive patient outcomes.
Where: Surrey based
Your Key Deliverables:
- Home-Based Care: Safely administer therapeutic interventions in line with NHS projects and training, ensuring a seamless transition for patients from hospital to home.
- Risk Management: Identify and report risks and incidents promptly, utilizing both HAH and hospital-based systems as per contractual requirements.
- Record-Keeping: Maintain accurate and contemporaneous clinical records, logging them onto relevant clinical systems for comprehensive patient care documentation.
- Safety First: Take responsibility for the health, safety, and welfare of yourself and others, adhering to Health and Safety regulations. Report any accidents or incidents using relevant documentation.
- Adherence: Follow CQC/GDP/NMC/GPHC/ICO standards relevant to your role.
- Committee Participation: Chair or attend all relevant committees aligned with the scope ofyour role.
- Infection Prevention and Control: Stay informed and vigilant about all responsibilities related to infection prevention and control.
Your Skills and Knowledge:
- Holistic Assessment: Conduct accurate holistic assessments of patients' needs, following approved care plans and programs.
Therapeutic Interventions: Safely administer a range of therapeutic interventions within the agreed clinical pathways. Skills
Registration: RGN part 1 NMC Register (RSCN where appropriate).
- Clinical Experience: Acute care experience post-registration, including making independent clinical decisions and supervising others.
- Therapeutic Understanding: Strong knowledge of therapies and drugs relevant to our service offering, witha focus on managing patients with treated conditions.
